What is chronic care management?
Chronic care management (CCM) is a specific care management service that provides coverage for patients with at least two chronic disorders, facilitating an ongoing partnership with their individual care team. This program entails creating a comprehensive care plan, coordination of care between providers, medication management, and interactive remote communication and management (generally over the phone).
Chronic care management is offered at GI Alliance of Illinois and can be helpful for individuals as it pertains to ongoing health and wellness support. It also allows for increased access to the proper medical resources, enhanced communication with members of their care team, reduction in ER visits and hospital stays or readmissions, and increased engagement in their individual healthcare. To receive chronic care management in Chicago, IL, Peoria, IL, or Normal, IL, contact our team today.
Who is eligible for CCM?
All patients having two or more qualifying chronic health conditions that are anticipated to last 12 months, and the issues increase their risk of flare or decline, are eligible for CCM. Please connect with our gastroenterology group in Illinois to get started.
The Dr was brief, explained what he believed was happening with my body and what he suggested I do before I see him again for my next appointment.
